There is only one prepaid package - "ULTRA START PAKET", and the SIM costs KM 15.00 (€7.50), which includes €5.00 of credit.
To top-up, buy a scratch value card for KM 10, 20 or 50 (€1 = KM 2), and type *124*XXXXXXXXX# (then press the Yes key), where XXXXXXXX is the hidden number on your scratch card.
Only GPRS or EDGE available. Prepaid users might not be able to use EDGE at all, but only GRPS. The EDGE coverage is good, but not so in the Serbian parts of the country (northern border with Croatia, and eastern border with Serbia).
To activate the GPRS option, send a free SMS with the text "PRIJAVA" to 061 11 00. You may be asked to send your ID information somewhere (possibly type it in another SMS, I don't know, it's not specified on their website).
Price is € 0.75 for 1 MB of data (10 kB is minimum unit of data).

IP Address: 195.222.056.041 (needed for phones only)
PORT/PROXY: 8080 ili 9201 (needed for phones only, and depending on the model)
